| Under the supervision of the Coordinator Clinical Services/Diabetes Education is responsible for providing diabetes education. Assesses patients individually for learning needs and provides individual or group education. Evaluates patients understanding of and implementation of their diabetes treatment plan and their ability to self manage. Adheres to regulatory standards. Participates in departmental activities. Provides staff education. Communicates effectively with patients, staff and other health care professional.

Qualifications:
1. Licensed in the State of Michigan as a Registered Nurse.
2. Bachelor of Science in Nursing (BSN).
3. Certification as a Diabetes Educator, preferred (CDE).
4. A minimum of one year diabetes education experience and two years of clinical experience preferred.
