The incumbent is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coaching program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing, and Whole Health clinical care elements. Coach support clients in mobilizing internal strengths, external resources, goal setting, action steps, and accountability. Develop self-management strategies with clients for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease. Participate in local, state and/or national professional organizations specific to health coaching, and represent health coaching in appropriate hospital committees. Contact eligible participants for enrollment and explain the health coaching process. Utilize motivational interviewing and health coaching techniques and processes, learned through prerequisite health coach training, to assist members in developing a realistic plan to accomplish their goals. Ensure applications are consistent with the current and planned infrastructure and data environments currently used through the knowledge of appropriate policies, directives, operating procedures. Develop and analyze relevant data that can be used to prepare analytical and interpretive reports and guides. Assist in the collection of whole health qualitative data from clients and creation of reports analyzing effectiveness of health coaching. Develops and conducts educational groups for Veterans, family members, and VA employees to teach health and wellness education, as appropriate and depending on departmental need (e.g. smoking cessation, diabetes management) Documents client treatment plans in a clear, concise, and timely manner using appropriate operating procedures and technology (e.g. CPRS, VISTA) Conducts outreach activities to inform Veterans, family members, and community organizations of Whole Health and available programs, providing in-servicing and education when needed. Maintains close working relationships with various program officials and supervisors across service lines throughout the medical center and with external Veteran organizations.
