Patient Sitter Float Pool FT Rotating Position Summary:
Working under the supervision of a Registered Nurse, the Constant Observer supports patient safety through monitoring and observation of assigned patient or patients. Provides continual bedside observation to assigned patient. Ensures patient safety. Intervenes as necessary to prevent physical harm to patients who might be at high risk to injure themselves or others due to confusion, delirium, self-destructive behavior, or a has a high predictive factors of falling. Works with others in the healthcare team to assure prompt intervention to maintain patient safety.
Remain at patient bedside at all times. Remove visible hazards and/or potentially unsafe equipment from the immediate environment. Report unsafe/broken equipment or conditions to immediate supervisor. Accompany patient to procedures/tests/therapy as needed. Promptly refer to supervising RN any signs or symptoms that may indicate a change in patientâ��s condition. Assist in calming or reorienting patient in cases of agitation or confusion. Communicate in a positive professional manner; develops and asks questions which open up the dialogue. Provide clear, direct and specific instructions or information to others. Listen with patience and empathy. Observe and respect of patientâ��s right to privacy. Maintain quiet environment. Adhere to professional conduct/dress. Maintain orderliness of patient rooms. Assists patient with basic activities of daily living, i.e. hygiene, oral care, oral intake (unless directed to do otherwise). Provides for mobility i.e. ambulating, up to chair, range of motion and turning. Assists others with patient's care. Demonstrate initiative and willingness to assist. Greet visitors promptly. Demonstrate use of effective interpersonal skills and present with a positive attitude. Assist in training new Constant Observers. Promote awareness of differing cultural, social, and religious needs among patients, family and caregivers. Checks in with charge nurse for assignment. Obtains report from patientâ��s nurse prior to starting assignment to include reasons for constant observation, level of constant observation, precautions, level of awareness, communication status, mobility, dietary restrictions, safety concerns, status for visitors. Able to follow and understand assigned level of constant observation for patient; all points below may have additional requirements based on each of these levels. Maintains constant visual contact including going with patient to other departments for tests or diagnostics, while patient in bathroom, and when visitors are present. Does not leave patient unattended or out of view (constant observer will schedule breaks with the patientâ��s nurse or PCT and stays with patient until another constant observer comes to take their place while on break). When in room, keeps patient in view at all times or within armâ��s reach if directed to do so. Does not shut door to bathroom when patient using it, keeps patient in view or within armâ��s reach if directed to do so. Does not leave room to get supplies or anything else. Documents on constant observer flow sheet every 15 minutes or more often as nurse instructs. Calls for assistance when needed and per hospital policy. Keeps patient door open/partly open at all times unless otherwise directed by patientâ��s nurse. Maintains level of constant observation at all times. Keeps sharp/dangerous/contraband items out of room. Assists nurse to inspect all items brought in by visitors; nurse will instruct you if assistance is needed. Does not keep scissors on person. Does not have items around neck (ID badge, stethoscope). Does not allow patient to leave room unless the nurse allows it. Does not try to physically restrain patient, does not physically block a patient attempting to leave; follows at a safe distance, calls for help. Calls for help immediately if the patient leaves the room. Does not follow a patient without calling for help. Ensures hospital specific Code is called for this situation. Does not allow patient to leave area including to go to other departments unless nurse allows; if a transporter arrives to take patient off the area, asks nurse if this will be allowed. Assists patient in/out of bed and to/from bathroom as ordered activity level permits. Informs care team immediately if patient expresses desire to harm self or others. Maintains confidentiality of all patient information. Refrains from discussing patient and information with anyone other than staff assigned to patient or patientâ��s physician. Respects the patientâ��s rights with concern for safety as a priority. Listens and acknowledges patientâ��s feelings. Conveys compassion, empathy and understanding. Patient has control of the choice of TV station. Does not have personal electronics or anything that takes your attention from patient (iPod, CD player, Game boy, phone, books, magazines). Does not make personal phone calls, eat food, close eyes, sleep. Always has someone in the room to observe the patient before you take any breaks. Notifies the charge nurse for any issues/concerns. Promote quality service within a compassionate environment by treating each person with dignity and respect. Support organizational efforts to provide health care for all. Contribute to the development, implementation, and promotion of the entityâ��s strategic plan and the departmental/unit program or operational plans aligned with the Networkâ��s Strategic Vision. Identify opportunities to achieve the vision and participate in the development and implementation of unit objectives and action plan tasks. Respond to organizational changes required to achieve strategic objectives. Perform duties in a safe manner, utilizing proper safety techniques and tools. Maintain patient confidentiality, and safety. Apply Infection Control Standards in all activities. Conduct self in a professional manner by respecting and maintaining confidentiality and privacy of patients and families at all times. Accept direction and delegation in a positive professional manner. Communicate in a positive and respectful manner to patients, families, and all staff. Utilize established modes of communication to stay informed on unit/hospital issues. Resolve conflict in a direct one to one professional manner. Assist patient care staff by observing the assigned patient during restraint use. Protect patient from harming self or others. Maintain the dignity, privacy and respect of all customers. Protect patients from harm and report risk management concerns. Identify location of Hazardous Communication Manual(s) and identify use of Material Safety Data Sheets (MSDS); Describe the procedure for handling spills involving hazardous materials and waste; Define the acronym RACER; Respond with appropriate phone numbers to call to report emergencies; Describe location of Emergency Preparedness Manual and describe primary component. Identify location of Unit Safety Plan Manual and describe the evacuation plan for the specific environment. Identify the location of fire pull-stations, hand-held fire extinguishers, and emergency power outlets for the specific environment. On going interactions and communication with patients, visitors, volunteers, other employees and management. Maintains a cohesive working relationship with other individuals in the patient care environment. Position is based at Carondelet St. Maryâ��s or Carondelet St. Josephâ��s Hospital. Works out of the central resource pool and receives assignments from the staffing specialist. Accountability to perform role in a respectful, professional manner.
THE PATIENT SITTER FLOAT POOL FT ROTATING CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.
EDUCATION Preferred: High School Diploma/Equivalent.
WORK EXPERIENCE Required: Must possess effective interpersonal skills. Must be able to follow written and verbal instructions. Must be able to work variable shifts, holidays and/or weekends. Must be able to transport self to alternative work sites. Within 90 days of hire completes all .Edu courses on duties of the Constant Observer and the .Edu courses on restraint and seclusion. Completes hospital approved training in managing patient behaviors and de-escalation techniques. Constant Observers providing patient care must have completed all applicable Competencies within 90 days. Preferred: Knowledge of hospital environment desired. Previous hospital and/or health care related experience preferred. Bilingual, English/Spanish helpful.
