The Social Work Care Manager is assigned to the Primary Care - Mental Health Integration (PC-MHI) Program with responsibilities to address the psychosocial needs of eligible veterans. The incumbent is responsible for a broad range of skills and services including completing psychosocial history and assessments, individual, couples and group counseling, case management, as well as the coordination of treatment plans for veterans to ensure seamless transition of care. **This is an OPEN-CONTINOUS ANNOUNCEMENT. The initial cut-off date for referral of eligible applications will be July 26, 2021. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled.** The PC-MHI Social Work Care Manager possesses knowledge and experience in the relationship between medical and psychological aspects of health and disease. Incumbent possesses knowledge in diagnostic issues related to psychological disorders resulting from or intensified by injury or disease. The Care Manager possesses specialized knowledge of evidenced based treatment for general behavioral health problems (e.g. depression and anxiety) and areas of behavioral medicine (e.g. chronic pain, obesity and sleep problems). Duties include, but are not limited to: Conducts psychosocial assessments to determine needs of veterans and family members. Provides a full range of social work services within commonly accepted standards of social work practice which includes individual, family and group counseling, discharge planning, case management and other services as appropriate. Carries pager during normal business hours and respond to same day consults when possible. This includes emergency evaluations requested by primary care providers and complete suicide evaluations with depressed patients. Provides timely and succinct feedback to primary care providers regarding findings and recommendations. Utilizes evidenced based therapies to address brief, problem focused issues. Learns the Behavioral Health Laboratory software program for interviewing participants. This includes determining the appropriate level of care need for patients not in treatment and the ability to understand the variety of clinical options available through the medical center and local community. Actively participates in the treatment planning process with other disciplines. Provides psycho-education and direct advice regarding symptoms of depression, anxiety and alcohol misuse to patients, as well as other health related behaviors (diabetes, hypertension, etc.). This includes the development of an appropriate treatment plan for patients and the ability to communicate that plan to the patient's provider. Provides social work consultation to Primary Care providers, other Behavioral Health clinicians and team members in the development of individual treatment plans. This includes coordinating treatment and communicating with other services to facilitate continuity of care for Veterans. Work Schedule: Monday-Friday, 8:00am to 4:30pm Work Location: One position will be located at the Omaha VA Medical Center. The second position will be located at the North Platte VA Clinic.
