This position administers community care coordination activities within clinics and affiliated medical centers.ESSENTIAL FUNCTIONS OF THE ROLEDevelops care coordination strategies to ensure achievement of short and long term goals including cost-effectiveness, improved outcomes, high patient satisfaction and reduced utilization of hospital care for the populations served.
