The School of Medicine, Ian M. Burr Division of Pediatric Endocrinology and Diabetes is accepting applications for a full-time Pediatric or Family Nurse Practitioner (NP). This Nurse Practitioner will be credentialed as an independent billing staff provider and VMG member. This individual will work in both the Children's Diabetes Program and the Pediatric Endocrinology Program in diagnosing and treating children with endocrine disorders. The NP serves as a role model and leader in providing comprehensive care to the population, in collaboration with other members of the interdisciplinary team: nurses, dietitians, social work and child life and other clinic staff to achieve the mission of our clinical programs. Our diabetes program has as its mission: "to provide individualized instruction and emotional support to encourage and empower children and their families to manage diabetes with independence and confidence." Clinical faculty in the program are expected to perform a part of their outpatient work at off-site locations in Middle Tennessee.
The roles and responsibilities for this position include:
In the outpatient setting, the NP will obtain histories, complete physical examinations, and provide appropriate assessments and plans for patients seen in the pediatric endocrinology and diabetes clinics.
The NP will also provide, and coordinate follow up care for this patient population.
This NP will order and interpret laboratory tests and procedures and coordinate with physicians according to established protocols.
The NP will initiate and/or recommend medical drug therapies or other forms of related therapeutic treatments and procedures in accordance with approved protocols/policy for the management of other pediatric endocrinology patients.
The NP will be responsible for accurate and complete documentation of all pertinent clinical findings, assessments, and plans in the electronic medical record.
The NP will participate in case management which may include education, counseling, and referrals when medically indicated.
The NP will participate in quality improvement activities and perform other duties as assigned by the Division Director.
The Ian M. Burr Division of Pediatric Endocrinology and Diabetes at Children's Hospital is nationally recognized for our expertise, experience and research in hormone-related disorders in children. Key Functions and Expected Performances:
Assessment of Health Status
Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients.
Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
Assesses for risks associated with the care of acute and complex endocrine disorders including medication side effects, hypoglycemia, hyperglycemia, growth suppression, impaired nutrition, fluid and electrolyte imbalance, autoimmunity, and need for diagnostic procedures.
Coordinates daily clinic schedule with the attending physician team, presents patient report, collaborates in plan of care, and follows assigned patient population.
Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions.
Manages diagnostic tests through ordering, interpretation, performance, and supervision.
Formulates differential diagnoses by priority.
Diagnoses complications and orders appropriate interventions.
Formulates Plan of Care
Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs.
Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum.
Implements and modifies plan of care
Prescribes diagnostic strategies and therapeutic interventions pharmacologic, non-pharmacologic, and surgical, needed to achieve expected outcomes.
Manages further diagnostic tests through ordering, interpretation, performance, and supervision.
Maintains ongoing communication and collaboration with interdisciplinary health care team.
Facilitates and communicates with patient, family and staff to promote continuity of care across the outpatient continuum of care.
Documents/dictates key components of patient's progress via daily progress note, transfer, and and/or clinic note, where applicable.
Documentation is timely, meets compliance standards and captures visit acuity.
Demonstrates Professional Practice behaviors including: preceptor/mentoring, education and instruction of students, nursing staff, nurses, graduate and novice nurse practitioners,
Seeks opportunities for active engagement in research and the analysis of evidenced based practice.
Actively participates in Grand Rounds, APN council meetings, faculty and unit meetings, M&M presentations.
Maintains CEUs, and membership in a professional organization
Qualifications - External
Master's Degree (or equivalent experience) and < 1 year experience. 12 months of nursing experience preferred; Inpatient acute care advanced practice nursing experience desirable.
One-three year NP experience preferred
Prior experience in diabetes education/management is preferred
