SUMMARY OF POSITION:
Plan and conduct consultation and educational services for persons with diabetes and provide community health education in the promotion of health and control of the disease. Develop curriculum on health and act as a health care consultant by providing information on the management of diabetes for patients of all ages from infancy to geriatrics. Collaborate with other health care team members in information exchange for the planning and delivering appropriate patient care.
Licensed Dietitian Nutritionist (LDN)
KNOWLEDGE, SKILLS & ABILITIES-
Ability to adapt education to the individual and family needs.
Ability to stay current with trends in diabetes care and evidence around diabetes care.
Ability to collaborate with the entire health care team.
