The Anticoagulation Clinical Pharmacy Specialist is located at the Sacramento VA Medical Center. Ambulatory Care Clinical Specialist spends a minimum of 25% of his/her time in direct patient care activities which include participation on Primary Care Teams, and pharmacist-run Anticoagulation, Diabetes, Lipid/Risk Management, Medication Renewal, Smoking Cessation or Mental Health Clinics whereby the individual serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy. The incumbent provides professional training to PGY-1 Pharmacy Residents, as well as training for Pharmacy clinical clerkship students from Schools of Pharmacy at University of California San Francisco (UCSF), University of the Pacific (UOP), Touro University, and California Northstate University College of Pharmacy (CNUCP). The remainder of time is spent in clinical pharmacist dispensing activities as assigned. Work Schedule: Monday-Friday, 8:00am to 4:30pm Financial Disclosure Report: Not required
