Description Clinical Pharmacist - Discharge Care Center Your Role: The clinical pharmacist in the discharge care center will provide telephonic medication management services to patients who were recently discharged from Vanderbilt University Medical Center. The primary responsibility of this pharmacist is to provide support to patients and families and assure appropriate, safe, and cost-effective drug therapy during transitions of care. The pharmacist in this role will interact with patients remotely to provide medication reconciliation, patient counseling, disease management, and help patients access medications after discharge. Other responsibilities may include process and quality improvement and other duties as assigned.
