Health Coaches work with Veterans on identifying the Veterans' motivation for change, determining, implementing Personal Health Plans, and assessing progress in order to adjust plans as necessary. Health coaches provide group education and may also support staff adoption of healthy behavior change. Health coaches are integral to advancing Whole Health system initiatives, and this position reports to the Whole Health Program Manager.
