Duke Primary Care

Duke Primary Care Duke Primary Care is a broad network of community-based clinics that offer family medicine, internal medicine, pediatric, and urgent care services. With 400 providers in nearly 50 locations, Duke Primary Care is the largest primary care network in the greater Triangle. Our network saw more than 750,000 patient encounters in fiscal year 2021.

Our Duke Primary Care clinics provide personalized care for adults and children, from sick visits to preventive care, health screenings to immunizations, and annual exams to chronic disease management. Select primary care clinics also provide behavioral health, diabetes education, pharmacy, and other population health services. Duke Primary Care clinics are accredited and designated a Primary Care Medical Home by The Joint Commission.

Our Duke Urgent Care centers offer evaluation and treatment for minor illnesses and injuries 12 hours a day, seven days a week when a patient’s primary care provider is not available. Services also include certain physical exams and vaccinations. A pediatric specialist is available at select locations. Patients can walk in to one of our nine urgent care centers located throughout the greater Triangle or can schedule a virtual visit to receive care from the comfort of home.

< p> Level IProvide psychosocial assessments, diagnosis, and treatment,as well as discharge planning to and consultation about patients andfamilie s to assist them and the health care team in coping withpatient's hospit alization, illness, diagnosis, treatment, and/or lifesituation, includin g emotional, mental, and substance abusedisorders.Level IIFunction in a supervisory role as a teamleader in addition to the duties and responsibilities of Level I.

Work Performed

Level IProvide ps ychosocial assessments of patients and familiesto identify emotional, so cial, and environmental strengths and problemsrelated to their diagnosis , illness, treatment, and/or lifesituation.Formulate, develop, and imple ment a comprehensivepsychosocial treatment plan utilizing appropriate cl inical social worktreatments and interventions. Interventions may includ e crisisintervention, brief and long-term individual, marital, family an d/orgroup therapies as well as grief and bereavement work.Screen,identif y, diagnose using DSM nomenclature, treat and manage mentalhealth and/or substance abuse problems in patients and family members.Provide trainin g to other health care professionals in same. Mayperform these functions independently or as part of a team.Assist with screening, identificatio n, diagnosis, managementand treatment of victims of abuse, neglect, dome stic violence, rape,etc. Provide training to other health care professionals insame.May participate in on-call or after hourscoverage.Provide co nsultative services to health care teammembers within scope of care defi nitions as needed.Maintain aworking knowledge of relevant medical/legal issues that impact onpatient care, e.g., advance directives, child and e lderabuse.Provide education to patients and families around issuesrelate d to adaptation to the patient's diagnosis, illness, treatmentand/or lif e situation.Participate in multi-disciplinary healthcare teams andprovi de leadership in representing clinical social workperspective/liaison wi th patient and family.Provide teachingand training for students, staff, and faculty from social work andother disciplines.Contribute to the acad emic mission of themedical center andhealth system by participating in academicactivities such as committees, conferences, publications, teachi ng andresearch.Maintain working knowledge of and liaison withcommunity a gencies and resources.Arrange, procure, andcoordinate patient/family pre and post hospital needs.Knowledgeof hospital, medical center and/or hea lth system resources to accessand provide for patient care needs.Documen t assessment, plan,interactions, and interventions according to departme ntal, hospitaland/or health system guidelines and standards.Maintain rec ordsand statistics in accordance with department, hospital, medical cent erand/or health system policies.Attend and participate in staff,committe e, department, and other administrativemeetings.Participate in departmen t, hospital, medical center,and/or health system continuous quality impr ovementefforts.Participate in regular supervisory conferences; keepcurre nt with social work and health care developments and seek toincrease fur ther enhancement of job related knowledge.LevelII Perform all of the dut ies and responsibilities of LevelI.Provide clinical and administrative s upervision for socialworkers and clinical social workers on the team.Lea d teammeetings and manage coverage.Participate in hiring andperformance management processes.

Knowledge, Skills and Abilities

Level I Adherence to the practice standards andethical guidelines of NA SW and the NC Certification Board for SocialWork. Excellent verbal and w ritten communication skills. Asolid knowledge of clinical social work pr actice. Excellentassessment, interviewing, and counseling skills with ex pertise inmental health and substance abuse areas; familiar with psychop athologyand with range of therapeutic interventions includingpsychopharm ocotherapy; and if working independently, must know when torefer for fur ther medical assessment. Ability to work effectivelyand autonomously in a self directed role. Highly collaborative withstrong interpersonal and team building skills. Ability toprioritize multiple work assignments and manage time efficiently.Flexibility in accepting diverse work assignmen ts and managing stressrelated to change. Basic computer skillsLevel II S ameas Level I plus strong supervisory and leadership skills. Abilityto r esolve/manage conflict. Ability to help implement departmental,hospital and/or DUHS initiatives.

Minimum Qualifications

Education

Level I and Level II Master's degree in social work from an accredited school of social work .

Experience

Level I Level II Two year of post internship social work experience in a health care setting or social service agency. Two years recent post master's experience in a clinical setting preferred. Same as Level I plus five years post master's experience in a health care or social service agency. Experience in leadership and supervising.

Degrees, Licensures, Certifications

Level I and Level II Current licensure as a licensed clinical social worker (LCSW) by the NC Social Work Certification and Licensure Board.
