VA New England is leading the VHA's Primary Care initiative in the Patient Aligned Care Team (PACT) model embedded in an innovative, Whole Health oriented Primary Care Delivery System. The Clinical Pharmacy Specialist (CPS) in Primary Care is assigned in the Primary Care service line functioning as a specialist in medication therapy management. The CSP functions at the highest level of clinical practice as a provider with prescribing privileges as outlined in the scope of practice.
