Registered Nurse Case managers are directly responsible to designated supervisors in the assigned areas. They are responsible for the provision of continuity of care, access, utilization management, clinical guideline implementation and monitoring and performance improvement in the area assigned. This includes overseeing and participating directly in the care of assigned groups of patients both inpatient and outpatient and working in an interdisciplinary setting.
