The VA Western New York Health Care System, Buffalo Medical Center, is currently recruiting for one Registered Nurse who will serve as a Home Telehealth Care Coordinator. The Nurse uses the nursing process and evidence-based practice to collaborate with the core interdisciplinary team in the Home Based setting.
