The role you’ll contribute:
Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions.
Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.
Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardiometabolic conditions across the lifespan.
Engages in lifelong learning and serves as a role model of professionalism.
Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.
Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.
Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’ assessed needs and goals and promotes successful self-management.
Documents all individual contacts/visits in the Electronic Health Record and outcomes data base according to the guidelines in a timely manner.
Collaborates, advocates, and confers other members of the diabetes care team in developing person-centered diabetes plans.
Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.
Collaborates with community partners to establish and maintain on-going support options.
Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.
Schedule work times to assure that the professional dietetic service needs of the facility are met.
Assist dietetic supervisor with dietary guidelines.
Integrate the dietetic aspects of patient care through communication and sharing of specialized information with the medical and nursing staff and contributing pertinent information to patients care plans.
Counsel inpatients and outpatients concerning regular and/or therapeutic diets and interpreting their needs for such diets.
Make appropriate communication to nutrition services staff regarding changes and recommendations.
Assist in the development and implementation of our written dietary policies and procedures.
Devote such time, energy, and skill necessary to maintain high quality dietary services.
Maintain the confidentiality of patient information as established by our facility’s polices and procedures.
Stay abreast of all other responsibilities required of a registered dietitian as set forth in any federal or state laws, statues, or regulations as enacted or as may be enacted or amended for hospitals and clinics.
Follow the duties and responsibilities as outlined in the established policies and procedures.
Develop and assist in developing classes and materials pertinent to group needs (i.e. weight loss, diabetes, cardiac, etc).
Other related and reasonable duties as assigned. Fulfill needed positions within the department as necessary and complete other assigned duties as requested by facility.
Qualifications
The expertise and experiences you’ll need to succeed:
Bachelor's Degree Dietetics, Nutrition or Food Science. Required
A Member of the American Dietetics Association Preferred
Past work experience within a hospital dietary department in the area of clinical care. Preferred
Diabetes self-management education and support experience Preferred
Previous experience Preferred
Has at least a Bachelor’s degree from an accredited college or university with major studies in nutrition,
RD - Registered Dietician Required
RN - Registered Nurse - State Licensure and/or Compact State Licensure Preferred
Valid license based on location of practice as a Dietitian (must be registered by the Commission on Dietetic Registration) Preferred
