Position SummaryThe Nurse Practitioner (NP) is an expert in diabetes care. The NP is responsible for the delivery of diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis, and reporting of clinical, financial, and satisfaction outcomes), and process design (leading initiatives to facilitate practice-based and organizational change).
