Position SummaryThe Nurse Practitioner (NP) is an expert in diabetes care. The NP is responsible for the delivery of diabetes care and education that involves compliance with evidence-based clinical standards, outcomes measurement (tracking, analysis, and reporting of clinical, financial, and satisfaction outcomes), and process design (leading initiatives to facilitate practice-based and organizational change). As a member of the glycemic management team, the NP provides direct patient care in the outpatient and community setting through implementation of standardized procedures, collaboration with the medical team to establish priorities, and assessment of solutions for patients to receive exceptional care.
