Responsibilities
Provides nutritional care to patients
Assess patient nutritional needs. Complete referral data collection, interview, history form, prioritize needs
Interprets, negotiates, calculates, and executes MD orders for modified diets.
Develops and implements nutrition care plans.
Provides written and oral education to patients and families as ordered by an MD for insulin initiation, meter instruction and oral agent instruction.
Records and documents appropriate information in medical records. Processes and records patient charges for services.
Evaluates patient skills and cognitive understanding by observation, discussion, study guides and follow-up calls as indicated.
Develops, reviews and revises patient instructional material.
Provides up to date to the community.
Responds to phone calls from the community.
Communicates with other departments and disciplines within the practice.
Provides for own professional growth and development.
Attends CME programs on diabetes, prenatal nutrition, or other related topics.
Maintains diabetes acumen by regular review of publications, research studies, and literature searches.
Participates in local diabetes education meetings.
Updates statistical data on diabetic patients monthly or as requested.
Qualifications
Registered Dietician, Licensed Dietician, or Certified Diabetic Educator license required.
CDCES Certification is strongly preferred.
Education Level: Bachelorâ™s degree from a four-year college or university
Experience Years: 1-2
Experience Industry: 3-5
Equivalent combination of relevant education and experience will be considered.

The pay for this position ranges from $59,000.00 - $105,500.00 per year and is based on years of relevant experience.
