POSITION SUMMARY Assists the surgical team to provide patient care for assigned patients under the direction and supervision of the Registered Nurse. Demonstrates increasing knowledge of physical and technical requirements of surgical intervention: Moderately complex procedures, advancing knowledge of instrumentation &#xe2;?? specialty sets. Demonstrates increasing knowledge of anatomy and physiology. Ability to scrub more than one specialty proficiently. Willingness to assist circulator. Responsible for maintaining effective communication skills. Participates in monitoring and maintaining resources in specific department. Understands and contributes to the vision/mission and goals of the department. Works in a secure department with access to areas where medications are stored. Handles and labels medications on the sterile field under the direct supervision of licensed personnel. RESPONSIBILITIES Assist the surgical team before, during, and after surgery.  Participate in &#xe2;??time-out&#xe2;?? and follows Universal Protocol Policy. Place equipment and supplies in the operating room and arranges instruments according to preference cards. Hand instruments and supplies to surgeon, holds retractors, cuts sutures, and performs other technical tasks as directed by surgeon during operation. Identify problems/concerns on the nursing unit and assists in developing resolutions in a positive, constructive manner. Make a positive contribution to the morale of the unit/section/department.  Participate in performance improvement teams/activities. Demonstrate ability to provide appropriate care to patients of all ages. Anticipate supplies and equipment needed by utilizing surgeon&#xe2;??s preference card. Understand and demonstrate principles of asepsis.  Use sterile technique when opening supplies and throughout procedure. Review anatomy and physiology involved.  Review operation of needed equipment for procedures. Assist with proper positioning of patient. Demonstrate Intra-operative knowledge and ability.  Keep accurate count of needles, sharps, and instruments according to policy. Use proper technique for scrubbing, gowning, gloving, and draping. Review operation of equipment and supplies as personal needs dictate to remain current in ability to use and operate equipment and supplies. Avoid unnecessary conversation during procedure. Organize in preparation for end of case and assists in clean up and turnover of rooms. Dispose of all sharps, biohazardous waste items appropriately. Handle specimens properly. Demonstrate knowledge of principles and proper use of various sterilization types and document cycles appropriately. Report equipment malfunctions to manager. Use time management and flexibility in organizing and prioritizing workload to provide optimal service to the department and the network. Level 2 &#xe2;?? Proficient  Participate in hospital safety and emergent programs. Guide the practice of support staff to achieve the patient&#xe2;??s functional goals. Level 3 &#xe2;?? Subject Matter Expert  Participate in performance improvement teams/activities and quality improvement measures within the department. Serve as a preceptor/mentor for students and staff, as appropriate. Act as a resource to promote occupational therapy within Carondelet. Cooperate with faculty of affiliated schools or health programs to assist in providing an optimum learning environment and experience for students in the department.   Qualifications: EDUCATION Required:  High School Diploma or General Education Diploma (G.E.D.) Certificates Required:  Current CPR - AHA BLS required Preferred:  Successful completion of Surgical Technologists course from an accredited facility. WORK EXPERIENCE  Required:  Minimum of one year experience. Preferred:  Knowledge of hospital environment desired. Previous hospital and/or health care related experience preferred. RESPONSIBILITIES In collaboration with the RN, provide direct patient care by following the established plan of care. Assist in collection of data for an objective assessment, i.e. vital signs, 12 lead, EKG&#xe2;??s, and blood draws. Demonstrate knowledge of and ability to assemble and maintain Endoscopy lab equipment including but not limited to manual cleaning of endoscopes, reprocesser operations, packaging of equipment for sterilization, and biological/chemical monitoring. Malfunctions must be reported via chain of command. Assist the physician and/or RN during the procedure with Endoscopy equipment, obtaining biopsies and positioning of the patient. May assist with applying pressure to the abdomen as directed by the physician. After appropriate training, discontinues peripheral IV catheter under the direction of licensed personnel. Maintain a clean and safe patient care environment, implement and follow infection control procedures according to hospital and unit policies. Demonstrate knowledge of endoscopy guidelines for cleaning and disinfecting of equipment. Check procedure carts for necessary items prior to the start of the case, assist with room turnover, and maintain current knowledge of physician preferences, supply items, new equipment and procedures. Demonstrate follow through regarding equipment needs, special requests, ordering of supplies. Maintain patient confidentiality and privacy at all times (HIPPA requirements). Respond to staffing needs to ensure continuous coverage to facilitate uninterrupted workflow. Demonstrate ability to prioritize operations. 								<description>Round Rock, Texas,  JOB SUMMARY The Health Coordinator 1 facilitates patient continuity of care with the healthcare team. This position is responsible for utilizing available resources to ensure patients receive care that is continuous, comprehensive and coordinated across the care continuum. ESSENTIAL FUNCTIONS OF THE ROLE Reviews reports to identify patient with past due adult preventative, diabetes and other identified clinical services; reviews the patient&#39;s electronic health record. Analyzes and compiles reports to support care coordination activities.  Facilitates care coordination with multiple teams, disciplines or external resources as required. Identifies and facilitates referrals to community resources (i.e. food, transportation, medication assistance, caregiver needs) as requested. Contacts patients to recommend needed services, schedules office appointments and pends orders and referrals for providers to sign. Addresses patient and family concerns.  Assists patients with self-management through approved, evidence-based education, telephonic engagement, encouraging and supporting patient adherence to their care plans as requested.  Monitors the patient transition from hospital to home and back to primary care physician (PCP). Contacts the patient to ensure completion of follow up activities. Notifies the PCP and clinical office staff of issues pertinent to patient care. Participates in departmental committees and quality improvement initiatives as appropriate. KEY SUCCESS FACTORS Medical Assistant certification preferred. Strong written and verbal communications ability. Strong attention to detail, with time management skills. Excellent listening, interpersonal and communication (oral and written) skills, and professional, pleasant and respectful telephone etiquette.
