Care Coordination and Transitions of Care       Utilizes a patient and family-centered approach.   Collaborates with other team members.   Empowers patients and caregivers to actively participate in their care.    Provides a clear and feasible management plan at discharge based on clinical needs, patient preference, goals, and abilities.    Evaluates the ability of the patient to obtain diabetes supplies and medication.        Ensures that a follow-up appointment for diabetes care is scheduled prior to discharge.    Obtains referrals to community resources to continue diabetes self-management education.       Communicates the status of diabetes self-management education, plan of care to the next provider and/or site of care.   Identifies and mitigates risk factors for readmission.         Collaboration       Actively participates and/or communicates with the system inpatient diabetes steering committee to ensure consistency in curriculum, tools and data collection.
