The VA Northeast Ohio Healthcare System is recruiting for an Registered Nurse (Case Manager). The Registered Nurse (Case Manager) will function in Medicine Service. This staff nurse position is located within Medicine Service, Endocrinology Clinic at the Louis Stokes Cleveland VA Medical Center. The Endocrine Care Manager Registered Nurse demonstrates the delivery of comprehensive nursing care through collaborative strategies with nursing, physicians, care providers, and other disciplines. The Endocrinology RN Care Manager uses the nursing process and evidence-based practice to collaborate with the Endocrinology Care Team providers and the expanded healthcare teams (family/caregiver, internal and community based services and disciplines involved) in developing the patient-driven endocrine care plans. The RN Care Manager is responsible and accountable for individual nursing practice and determines the appropriate delegation of task consistent with the nurse's obligation to provide optimal care (ANA Code of Ethics for Nurses). Provides a full, range of nursing care to patients with complex care needs utilizing the nursing process based on the ANA Standards of Nursing Practice. The RN will also serve as an adjunct to the clinical staff and facilitate and oversee clinic flow, which includes providing patient and nursing education, as well as checking in and rooming patients. The RN will huddle with members of the team as requested. Daily assignments will be given for new or unusual work as specific needs of the section arise. Instructions will include objectives and specific deadlines. It is the responsibility of the incumbent to be able to prioritize tasks and to be able to use professional judgment as to how to complete a given task. It is expected that the incumbent will not only collect requested data, but also analyze, interpret, and summarize data in order to achieve the long- and short-term goals of the section. Some additional duties may include but are not limited to: Returning patient clinical phone calls and voicemails. Endocrine currently receives approximately 20-25 patient clinical calls a day Educating post-visit recommendations and laboratory and pathology reports to patients Monitoring and replying to secure messaging in a timely manner Performing Endocrine based dynamic testing such as IV Cortisol Stimulation Tests Coordinating diagnostic and therapeutic radioactive iodine testing with Nuclear Medicine Providing clinical preparation for thyroid biopsy clinics a navigation for patients with positive biopsy results Providing patient education and assisting with troubleshooting software issues for diabetes devices in coordination with IT Assesses learning needs of patients and families, and plans, implements, evaluates, and documents individual teaching plans according to established protocols. Promotes the services and programs available. Participating in modes of communication both formal and informal effective , timely and bidirectional with all team members and care delivery including, but not limited to, secure messaging, telephone care, view alerts management, shared medical appointments, clinical video telehealth visits, face to face visits, etc. Entering orders in CPRS for tests/prosthetic items per approved standardized RN care management protocols or PCP orders.
