| This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more Work Schedule: 8:00am - 4:30pm, Monday - Friday Financial Disclosure Report: Not required The incumbent will be an integral part of Geriatrics at VAPAHCS and will advocate for and participate fully in its academic programs, including education and mentoring. The incumbent will work in close collaboration with the GRECC, primary care leadership, nursing service leaders, social services, and Extended Care Service leaders. This position includes direct patient care and teaching activities such as establishing and attending in the Geriatrics Clinic, dementia telehealth clinics, and possible consultation on the hospital wards or Emergency Room at VAPAHCS if needed. The incumbent will be responsible for assessment and care of new and returning patients with multiple medical problems (e.g. dementia, hypertension, diabetes, ischemic heart disease, COPD), ordering diagnostics, consultations, medications and other therapies as medically indicated on an urgent need basis, meeting preventative health maintenance needs, completing all required VA computer documentation including clinical reminders and screenings. The incumbent will be primarily located at VA Palo Alto Health Care System. Primary functions of the position: Understands and follows VHA's "Mission Statement", "Code of Ethics", organizational policies, and structure. Establishes Geriatrics care protocols. Promotes and builds age friendly healthcare system through collaborative efforts Provides Geriatric consults, either in person or via telehealth, for Veterans at the medical center or Community Based Outpatient Clinics Helps develop geriatric focused clinicians in Primary Care. Works with GRECC to disseminate best practices and teaching tools. Provides primary care and specialty Geriatrics and dementia care using generally accepted standards of practice. The physician develops a plan of care for each patient, including complete history, physical examination, diagnosis, appropriate treatment and /or referral, uses all available resources in an appropriate and cost-efficient manner. Hospitalizes as needed any patient requiring in-patient care. Provides primary care during the hospitalization when on call. Confers with consulting physicians, midlevel providers, nurses, support staff, patients, and families concerning treatment and care of patients. Makes appropriate specialty and sub-specialty referrals, while maintaining continuity of care as appropriate. Accepts on call coverage responsibilities as scheduled. Participates in health promotion /disease prevention activities. Stresses the importance of preventive health care measures. Participates in quality improvement /assurance activities. Duties include the use of automated systems (i.e. VISTA) to enter clinical orders, ancillary services, and prescriptions; completion of workload accountability documents (i.e., manpower accountability documents (i.e. CPRS) on a weekly basis. Participates in the clinic performance improvement and risk management activities as necessary, to include, but not limited to, peer review and chart review. Attends other mandatory training as directed by supervisor or command. Ensures the highest standards of professional practice and ethics are maintained; that established operating policies are adhered to; and that medical records are properly completed and maintained.
