Right now, we are looking for a proactive CDE with strong clinical expertise on diabetes management, to promote diabetes-related literacy that will positively impact a self-care lifestyle through treatment and advocacy, and boost well-being in our culturally diverse population. What you will do in this role: As a driven and change-inspiring Certified Diabetes Educator, you will promote healthy eating habits through individual nutritional assessment, diagnostic therapy, and counseling services, to educate patients and their families with or at risk for diabetes on self-management. Collaborating with multidisciplinary teams, you will perform in-depth motivational interviews to identify prospective and fully-fledge diabetes; develop a treatment program based on evidence-based procedures and regulatory requirements and monitor adherence. You will also train and educate nursing staff; attend meetings, in-service training, and outreach activities; and participate in quality assurance/improvement monitoring and reporting initiatives.
