Banner Pharmacy Services provides innovative pharmacy services and coordinated patient care across the continuum leading to improved medication outcomes. In this position you will have the opportunity to work as an Ambulatory Clinical Pharmacist providing disease state management services while working under collaborative drug therapy management (CDTM) agreements. The Ambulatory Care Clinical Pharmacist works closely with providers to deliver population-based disease management, patient and provider education and collaborative drug therapy management through direct patient care and telephonic follow-up. The pharmacist actively manages medication therapy, completes medication reviews and collaborates with providers to optimize medication therapy, improve medication adherence, promote wellness, prevent disease, and improve outcomes. This position will support our ambulatory pharmacy services in Northern Colorado, either within our NCMC and McKee MedCare Clinics and/or within our Banner Medical Group clinics. Starting pay rate for this position is $52.57 - $87.62/hour based on experience and education. This is a full-time position; Monday-Friday from 8am-5pm Banner Pharmacy Services division provides excellent patient care across our entire continuum through convenient access, improved medication outcomes and innovative pharmacy practices. Banner Pharmacy Services will be recognized nationally as a clinical leader and trusted partner in proactively providing innovative pharmacy services through excellence in integration, care coordination, medication management and service to make a difference in people's lives. Banner is expanding its focus from being a hospital company to a clinical quality company focused on population health management. Clinical Pharmacy Services are also included in this robust new service line and are responsible for the management of the Banner formulary, development of standardized system wide clinical pharmacy services, and management of adverse drug event preventions. Clinical Pharmacy Services provides collaborative drug therapy management and educational interventions in patient care to optimize medication therapy, improve outcomes, promote wellness, and disease prevention. POSITION SUMMARY 5. Participates in ordering or prescribing, administering and monitoring of all medications utilized. Internal customers may include Patients, Nurses, Licensed Independent Practitioners, Administrators and other health care team members for the purpose of integrating services, improving patient care and ensuring effective communication systems. External customers may include local, state, and national pharmacy organizations, State Board of Pharmacy, colleagues and peers, educational institutions and pharmaceutical representatives. A degree in Pharmacy from an accredited College of Pharmacy. Completion of an accredited PGY1 pharmacy residency program or three years of previous clinical pharmacy experience in the ambulatory care setting. Possesses good written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.
